New Client Inquiry

After filling out this form, you will be prompted with a page that includes the forms that need to be properly filled out in order to move forward. New pets must have completed registration forms and pet clearance to be evaluated at the meet and greet. Upon completion of these forms, email them to HiloHolidayPet@Gmail.com and you will be called to set up a time for the meet and greet.

$50 Cancellation Fee & $30 Change of Date Fee

Checklist:

1. Fill out the New Client Inquiry Form above

2. Fill out Boarding Application and Waiver Agreement

3. Both you and your veterinarian fill out Vet Clearance

3. Email the completed forms to HiloHolidayPet@Gmail.com

4. Once your forms are approved, you will receive a phone call or an email to coordinate a time to come to the property for a meet and greet

5. Your pet will be evaluated by our team to ensure a good fit for our program
